Welcome to Wolverhampton Dental. We have a dedicated team that offer you the best and latest in General, Family and Cosmetic Dentistry. REGISTRATION

We welcome all new patients, both NHS and Private. Our registration process is very simple. All we require prior to your first consultation is for you to complete a Medical History and Registration Form which can be collected from our surgery or downloaded at the following link - Medical History Form. Please ensure you provide a full detailed list of all your medication.

PAYMENTS

At your first appointment, you will be expected to pay for your basic examination charge. Any further treatment that you may require and have requested will be discussed with you and you will be given an estimate prior to any charges. Payment for treatment will then be collected subsequently over the course of treatment - you will be informed how much is due each visit depending on your treatment plan.

We accept cash, and credit and debit cards except for American Express.

At present NHS Examination Fees are £18.50. If you are exempt from paying, we would ask that you bring your evidence of exemption to all visits.

NHS Exemptions

You do not have to pay for NHS dental treatment if, when your treatment starts, you are:

  • under 18
  • under 19 and in full-time education
  • pregnant or you've had a baby within the 12 months before treatment starts
  • staying in an NHS hospital and the hospital dentist carries out your treatment
  • an NHS Hospital Dental Service outpatient (although you may have to pay for your dentures or bridges).

You can also get free NHS dental treatment if, when the treatment starts or when you're asked to pay:

  • you're included in an award of Income Support, income-based Jobseeker's Allowance, income-related Employment and Support Allowance or Pension Credit guarantee credit or Universal credit (check entitlement after October 31 2014).
  • you're named on, or entitled to, a valid NHS tax credit exemption certificate
  • you're named on a valid HC2 certificate

If you're named on a valid HC3 certificate, you may not have to pay for all your NHS dental treatment. HC2 and HC3 certificates are issued under the NHS Low Income Scheme.

You will not be exempt from paying for NHS dental treatment if you receive one of the benefits below when paid on their own:

  • Incapacity Benefit
  • contribution-based Jobseeker's Allowance
  • contribution-based Employment and Support Allowance
  • Disability Living Allowance
  • Council Tax Benefit
  • Housing Benefit
  • Pension Credit savings credit

EMERGENCIES

If you have a dental emergency and you are a registered patient with us, we will endeavour to see you with 24 hours where possible. If you have an emergency out of hours that cannot wait till the surgery re-opens, please call NHS Advice on 111. NHS 111 is open 24 hours a day, 7 days a week. You may also call the practice phone number on 01902 723 400 to be directed to any other appropriate services.
